Checklist

Step 1: I have the pharmacy's name and the state it operates fromFrom the medication label, shipping paperwork, or a direct answer from the provider. Without this, no later step is possible.

Step 2: I know whether it's claimed as a 503A pharmacy or 503B outsourcing facilityThe registration class determines which registers apply and the exposure to pending federal compounding decisions.

Step 3: State license verified as active on the board of pharmacy lookupSearch the state board where the pharmacy operates; confirm status and type match what you were told.

Step 4: Nonresident registration checked in MY state (if shipped from another state)Many states require out-of-state pharmacies shipping in to register with the destination state's board too.

Step 5: If 503B: found in the FDA outsourcing-facility registerRegistration is not an FDA endorsement — but a facility claimed as 503B that is absent from the register is a red flag.

Step 6: Searched FDA warning letters and recalls for the pharmacy's nameRecent findings about sterility or potency are disqualifying for injectable products.

Step 7: Checked branded product against current FDA counterfeit alertsCounterfeit Ozempic has twice reached licensed pharmacies; alerts list the exact lot/serial identifiers.

Step 8: Saved what I found (screenshots or PDFs, with dates)Terms and listings change; a dated record is what makes your verification usable later.

When you can't complete it

Step 1 is where most attempts end today: no provider we track names its dispensing pharmacy publicly, so completing this checklist usually requires asking — theguide includes the exact question to send. An incomplete checklist is information, not failure: each unchecked box is a specific thing you were unable to confirm about a product you would inject, and "I could not verify the supply chain" is a legitimate, sufficient reason to choose a different provider — or thebranded route, whose supply chain checks differ but exist.
