Pharmacy disclosure: the field every provider leaves blank

Most comparison sites publish pharmacy 'profiles.' We can't — because with 0 of 20 tracked providers naming their dispensing pharmacy, there are no verifiable pharmacies to profile. What we can publish is the disclosure record itself: who has said what, verbatim, with status labels, and what a complete answer would contain.

What a complete disclosure looks like

The register's standing question to every provider: "Which pharmacy or pharmacies fill prescriptions written through your programme, and is each registered as a 503A compounding pharmacy or a 503B outsourcing facility?" A complete answer names each pharmacy or facility, states its registration class (503A pharmacy or 503B outsourcing facility), and thereby becomes checkable three separate ways: against the state board of pharmacy where it is licensed, against the FDA's public register of outsourcing facilities, and against FDA warning letters and recall notices. Why the name matters more than the claim

"U.S.-licensed pharmacy" is a floor, not an answer. Licensure varies by state; compounding quality history varies by facility; and the pending 503B bulks-list decision means the registration class of your pharmacy determines your exposure to a live federal decision. Meanwhile counterfeit product has twice reached licensed U.S. pharmacies — so even "licensed" is a filter, not a guarantee, and traceability from your specific prescription back to a specific facility is the thing that makes every other safeguard usable. That chain starts with a name.

What we will publish per pharmacy, once named

When disclosures arrive, this board becomes a directory with one entry per named pharmacy or facility, each carrying: the name and operating state as disclosed; the registration class claimed (503A or 503B); the state-license lookup result with the date we checked it; for 503B claims, presence in the FDA outsourcing-facility register with the date checked; any warning letters or recalls found under the exact name, with dates; which providers route prescriptions through it, per their own statements; and the evidence status and capture date of every one of those fields — the same per-fact labeling the rest of this site uses. We publish the template now so providers know precisely what a complete answer unlocks, and so readers can hold us to filling it rather than summarizing it.
